Emotional Memory as a Pathway to Connection and Meaningful Experiences for People Living with Dementia

All people continue to experience emotional memories, or the memory of feelings, long after an event has passed. For those with dementia, their emotional experience of the world around them may be heightened. While they may no longer recall an event, they are still filled with feelings from the emotional memory of the experience. This retained emotional memory continues to impact the person's affect and mood for hours or days. This session will teach care partners how to have a powerful impact by generating positive emotional memories from experiences of connectedness, happiness, joy, love, humor and hope, thus facilitating well-being for the person.

In-Room Flexible Medication Administration: Making and Sustaining the Change

Many organizations have not been able to successfully transform the medication administration system and still have medication carts. For ten years, we have been administering medications in the residents' rooms and would not go back to the old way. Eliminating medication carts and administering medications in a resident's room can be accomplished. This has been one of the most successful changes that we have made on our journey toward person-directed care. We will share with you how we made this happen, the benefits, and methods of sustaining this change.

Survey Readiness in Culture-Changed Households and Neighborhoods

Federal regulations support the concepts of culture change. There have been modifications in the last few years that clarify how surveyors should approach the changes in the operation of a culture-changed home that are different from a traditional nursing home. This session will discuss strategies that households and neighborhoods may use to ensure survey compliance while providing resident-directed care.

Come Dance with Me: Bringing Dance Programs to Older Adults
ROOM: Heartland Café Theater
This presentation provides a model for a dance/expressive movement program which is fun, engaging and esteem-building for older adults, people with dementia and their caregivers. The structure is designed to incorporate the movements and offerings of all participants, regardless of physical or cognitive abilities. Suggestions for choosing music, props and themes and current research into the benefits of dance and the arts will be offered. Caregivers and staff who enjoy dancing and moving will leave with the resources to provide rationale for and implement such a program.

The journey to a person-centered model of care begins with wise leadership. Formal Leaders must grow themselves so that they can grow others. Reframing Home Care through Person-Directed Practices

Even someone's home can feel like an institution. By engaging "caregivers" and "care receivers" in a balanced, person-directed relationship, care partnerships can develop that transcend role and compartmentalized care services. Empowered care partner teams support the well-being and ongoing growth of Elders and everyone who collaborates with them. This session highlights the efforts of two home care organizations to adopt person-directed practices, one of which has developed a corporate strategy to engage multiple franchises. Participants will explore the value of applying person-directed principles to home care and how to apply these concepts to daily operations.

Trust The Experts: LEAN Management Principles in a Nursing Home Environment

This session will increase participants' knowledge of LEAN principles and staff engagement. Participants will hear from an interdisciplinary team (co-lead by Management and Direct Care Staff) from a large New York nursing home about their experience in utilizing LEAN tools to identify and reduce waste, increase efficiency and save money. Additional benefits include utilizing all levels of staff as organizational change agents and developing new leaders of unit-based teams.

Connected Living: Using Technology and the Internet to Educate, Entertain and Engage

The primary goal of our session is to educate and inform care partners about ways in which they can use standard mobile electronic devices (tablets, laptops, etc.) as engagement tools and therapy aids. We will also explore possible ways in which nurses and therapists can integrate technological solutions into their care routines, specifically for elders experiencing speech, language, and/or memory challenges as a result of neurological injury or progressive dementia. Our secondary goal will be to highlight how free exchange of ideas and techniques among therapists in regards to technological interventions can improve outcomes.

Living and Working in Eldercare Communities: Bridging the Generations

Today's long-term care communities are a living laboratory for the study of intergenerational relationships. Three and four generations are sometimes represented in an eldercare community with each generation emanating from a different time in history and differing values and definitions of "acceptable" behavior. The various perspectives and values can present challenges as well as opportunities, which can sometimes be overlooked. There is a need to find common language and common ground, to find synergies and appreciate strengths. Join us to take a realistic look at the challenges and benefits inherent in a multigenerational organization while reviewing the communication skills that support positive interaction.

Emotional Intelligence and Dementia Care: Building Caregiving Skills

Emotional intelligence (EI) is the ability to manage yourself and your relationships in order to fulfill your goals. EI includes self-management, empathy, managing relationships, influencing others, and having a sense of purpose and vision. These are essential skills needed to form relationships with and care for people with dementia. During this session, participants engage in learning exercises to strengthen one's emotional intelligence and develop dementia caregiving skills: like validating feelings; expressing therapeutic emotional responses when an elder with dementia is struggling or distressed; bonding strategies to increase cooperation during care; and empower decision making.

Engaging Staff in Individualizing Care - Part 2: How to Accelerate Performance Improvement

As long term care policy increasingly links quality of care and reductions in avoidable occurrences (e.g., hospitalizations) to reimbursement, providers of services have an even greater imperative to ensure good outcomes. Knowing each person's history, preferences, and routines provides information essential to guide delivery of good care. Without this information, staff can inadvertently cause iatrogenic declines – declines that are avoidable and not a natural progression of a person's illness or condition. Huddles are a mechanism for staff to come together to share their knowledge about residents so that the care team can make and implement a solid game plan for preventing declines, catching problems early, and intervening effectively to improve residents' quality of care and quality of life.

Culture Change: Fad or Movement? / Making the Message Work!

While there are still 1.6 million persons living in nursing homes across America, there are even greater numbers of older adults, many with dementia and other chronic challenges, living in assisted living and so-called independent living residences. Some think that culture change and Pioneer Network are all about nursing homes. We have been around for close to 20 years and many say that we are still on the fringe of the senior living industry, including assisted living, memory care and nursing homes. If this perception persists, should Pioneer Network and the culture change "movement" revamp how the concept is being marketed? Moderated by a well-known journalist in senior living issues, this session will challenge all of us by focusing on three key questions: What is the fundamental content of person-centered care? If we truly want to mainstream it, who needs to lead? How do we better tell the story to reach and impact the broader community?
